If your letter or document is for school, or an employer, then please ensure that you follow the instructions outlined by your particular employer, school board, region, organizations (ex. sports leagues, gyms, etc.) or institution. It is your obligation to provide us with the form you wish to be commissioned by our Commissioner of Oaths. If you cannot locate a specific form required, our office may provide you with a standard form declaration, however we cannot guarantee it will be accepted (extra fees applied for drafting). We do not provide legal advice on employment issues as per our disclaimer set out below, and it is your responsibility to ensure that you know the required documents accepted for your particular needs.

Upon booking your appointment and paying the required fees, you agree that we will not be held liable whatsoever for any loss of employment or other loss, whether financial or otherwise, in the event the employer chooses not to accept your notarized Covid-19 Exemption Letter. If you have any questions regarding your relationship with your employer, you should consult with an employment lawyer who may assist you in understanding your legal rights regarding vaccine exemptions or other matters related to Covid-19 and your employer. By engaging our services you are acknowledging that no legal advice has been represented to you about employment law or the legal rights affecting your employment, or regarding the use of your letter to any organization or institution.
